Adaptive Behaviour Assessment System Third Edition (Hong Kong) [ABAS-3 (HK)] can be purchased and used by registered ABAS-3 (HK) users only. Please follow the following procedures when you apply for registration as an ABAS-3 (HK) user.

Step 1: Complete and Submit the Application Form

Please complete the application form carefully in accordance with the on-page instructions (see detailed guidance below). After confirming that all information is accurate, click “Submit”.

The system will automatically send the unsigned application form as an attachment to your registered email address. Please download, print, and sign the document.

Step 2: Upload the Signed Document

After signing, scan the application form into a PDF file and upload it using the link provided in the email.

Step 3: Document Review

Our staff will review your submitted materials to ensure that all information is complete and accurate. If any additional details or corrections are required, we will contact you by email.

Step 4: Completion of Registration

Once your application has been approved, we will send your electronic certificate, the scanned copy of your signed application form, and your account credentials (username and password) to your email address.

At this point, your registration will be officially completed.

Section D: DECLARATION OF THE APPLICANT

I hereby declare that:

the particulars given on Section A are true and correct;

my proficiency in both spoken Cantonese and written Chinese is adequate for the administration of the ABAS-3 (HK) and thorough understanding of related materials;

the ABAS-3 (HK) is to be used by myself, or by individual(s) under my direct supervision ONLY for the purposes of research and professional training of psychologists or paediatricians;

the use of ABAS-3(HK) will be bound by my profession’s code of ethics.This includes keeping the ABAS-3(HK) confidential; in the event that ABAS-3 (HK) is purchased under my name by my employer (hereafter refer to as ‘Employer’), I understand that I am obliged to inform the Employer that ABAS-3 (HK) can ONLY be used by registered users;

in the event of my departure from the Employer, I shall inform the Employer that ABAS-3 (HK) must be secured in a locked place until the joint responsibility of managing ABAS-3 (HK) [with the Employer] is taken over by a registered user employed by the Employer;

I understand that the Publisher is authorized to review the registration status, which could be subject to suspension or revocation, of registered user on a need basis. It will not be accepted any liability for misuse whatsoever arising from professional/technical misinterpretation of ABAS-3 (HK).
